Massage Establishment

Prior to Starting

An application must be completely filled out and approved by the license inspector before you can lawfully engage in business with the Town of Marana.

Fees

$322 application fee

  • $200 application fee
  • $100 annual licensing fee
  • $22 background fee

Required Documentation

  • Completed Massage Establishment Checklist(PDF, 168KB)
    • Please type or print clearly on your application
    • Answer all questions completely if question is not applicable put NA in appropriate space
    • “Describe nature of business” must be filled out, please be detailed
    • Provide TPT number (Transaction Privilege Tax number)
  • Completed Massage Establishment License Application(PDF, 195KB)
  • Completed Fingerprint card
    • Fingerprints are available at Marana Police Department Tuesday 9:00-12:00 & Thursdays 1:00-4:00
  • Government issued picture ID
  • Current headshot (Taken by licensed inspector)
  • Articles of incorporation, articles of organization, or certificate of limited partnership
  • A site plan showing the configuration of the overall business premises and a floor plan containing the information*(MTC TITLE 9-5-9 paragraph 17)
  • Written proof evidencing the legally enforceable right of the applicant to have or obtain the use and possession of the tract of land on which the establishment is to be situated for the purpose option contract, lease option contract or other similar documents
  • Business license application & supporting documentation can be submitted via email to: Licensing@maranaAz.gov or:

Prior to Approval

The application will be routed to various town departments for their recommendations. Below are the departments and the issues they base their recommendation upon:

  • Police Department (Background check)
  • Legal Department (Compliance with the Town Code)

Renewal

$122 renewal fee (due 60 days prior to expiration)

  • $100 annual licensing fee
  • $22 background fee
  • $25 late fee if renewing after 60 days prior to expiration

    • Answer all questions completely if question is not applicable put NA in appropriate space
  • Copy of government issued ID
  • Completed Fingerprint card
    • Fingerprints are available at Marana Police Department Tuesday 9:00-12:00 & Thursdays 1:00-4:00
  • Current headshot picture of applicant to be taken by license inspector
